As per the step help:

Connects two calls together, using transfer capability of your telecommunications circuit.

This step only works with ISDN circuits that are configured to provide the 2B-Channel Transfer capability (sometimes referred to as "release link transfer"). This is available on the NI2 protocol variant.
PROPERTIES

Other port's call reference number:

In order to connect two ISDN calls together, you need to use the call reference number for each call, as provided by your ISDN circuit. TeleFlow knows the call reference number for the current port, however, you need to supply the reference number for the other port (i.e. the one that places the outbound call).

To obtain the call reference number, capture the value from the @SYS_CALLREF system variable.
SPECIAL HARDWARE CONSIDERATIONS

In order to use the Transfer Call step, your application must be connected to a 2B-Transfer capable ISDN circuit.

As to your specific questions:

One channel places the call, and once it gets a caller on the line, you must run the transfer call step on the channel/port that the original caller to your system was on.

It is operational. It is supported and has been tested on ISDN with the NI2 protocol variant only.
